For medical records, patients can request a copy or transfer their chart to another provider by reaching out to the internal medical records department. Optum Behavioral Care of Connecticut, P.C. and RICBT will serve as the custodian of their medical record. Patients can mail their request to OBC Medical Records Department at PO Box 15006, Minneapolis, MN 55415-0006, or call 877-204-2110, or email obcmedicalrecords@optum.com.
