Northwest Colorado Health (formerly Northwest Colorado Visiting Nurse Association) is a leading community health organization that has been supporting wellness in the area since 1964. We provide a range of health programs and services to residents of Routt, Moffat, and Jackson counties, covering all ages, incomes and cultures. Our services include Primary Care at Community Health Centers in Steamboat Springs and Craig, Home Health and Hospice services in Routt, Moffat and Grand counties, and a variety of Public Health programs. Additionally, our organization operates The Haven Assisted Living Facility and Community Center in Hayden and the Rollingstone Respite House in Steamboat Springs.
