Located at 320 Ouachita Avenue, Hot Springs, AR, 71901, the practice is accessible by appointment only, with hours from 5:00pm-7:00pm on weekdays and closed on weekends. Level Ground Counseling and Consultation accepts various insurance plans, including Ambetter, BlueCross and BlueShield, Federal BCBS, Health Advantage, Medicare, Optum, QualChoice, and United Medical Resources (UMR).
