At Lenape Valley Foundation in Bristol, our mission is to support members of our community struggling with mental health, substance use, intellectual and developmental challenges. We offer Intellectual Disability Supports Coordination to help individuals and their families assess their needs, create individual support plans, obtain services and supports, and monitor those services. Eligibility for our ID services is based on a diagnosis of either Intellectual Disability or Autism before the age of 22, as well as significant limitations in daily living. Our goal is to help all members of our community access the resources they need to reach their personal aspirations and live a quality life.
