At Diamond Willow Assisted Living in Little Falls, we take pride in our person-centered care model, which is uniquely different from most diagnosis-based care systems. We believe that bigger is not always better and strive to create an intimate setting, with fewer residents per suite. This allows us to have smaller communities, promoting our person-centered care model in a calm and comforting home-like environment. Our small community settings comprise 8-16 residents per home and are provided with the ultimate care by an integrated team of direct care staff, onsite leadership team, and site-based registered nurses. We are able to provide one-on-one attention in a more positive, healthy atmosphere to each and every individual, and offer industry-leading staffing ratios of 1:5 during awake hours and 1:13 overnight.
