Community Care | Milwaukee County (Layton PACE & Partnership Office) is a nonprofit organization with over 40 years of experience serving the long-term care needs of older adults and adults with disabilities. Our Medicare/Medicaid programs coordinate and deliver supportive services to help those in need live with dignity, independence, and confidence.
