Lenape Valley Foundation, located in Doylestown, PA, is an organization with a mission to help members of our community who are facing mental health, substance use, intellectual, or developmental challenges. Our Intellectual Disability Supports Coordination program is designed to assist individuals and their families in assessing current needs and developing Individual Support Plans, locating and coordinating services and supports, and monitoring those services. Eligibility for ID services is based on a diagnosis of Intellectual Disability prior to the age of 22, or a diagnosis of Autism prior to the age of 22, alongside significant limitations in activities of daily living.
