At Mosaic in Phoenix, we strive to make a difference in the lives of people with development disabilities. We offer four distinct programs to make this possible: Host Home Services, Early Intervention Program, Residential Program and Home and Community Based Services. Our Host Home Services offer 24-hour home settings for children, teens and adults, Valley-wide. Our Early Intervention Program works with parents to provide in-home therapeutic activities to promote healthy development for young children, birth to three years old. Our Residential Program provides 24-hour support in six attractive adult residential homes in Central/Northwest Phoenix, where residents learn practical life skills and receive personal care. Finally, Home and Community Based Services offer customized assistance in the privacy of your home, Valley-wide. Respite, habilitation and attendant care are available through this program. We are dedicated to providing quality care and support to those with development disabilities and are committed to making a positive difference in their lives.
